Joining online is quick, straightforward and completely secure.
  1. I undertake to inform Youth Action of any changes in the information given on this form.
  2. I hereby give consent for any activities/ projects for me to participate with Youth Action Ltd.
  3. I give permission for personal pictures or quotes taken on any project with me to be used for ‘Youth Action Ltd’ brochures, information packs, leaflets, displays and images on our internet website. I also consent for Youth Action Ltd to share and distribute with other organisations/ companies the above on my behalf.
  4. I consent to any emergency treatment necessary during the course of the activities. I therefore authorise the group leaders to sign, on my behalf, any written form of consent required by the hospital authorities should a surgical operation or serum injection be deemed necessary and provided that the delay required to obtain my signature might be considered in the opinion of the Doctor or Surgeon, likely to endanger my health and safety.
  5. I undertake to inform Youth Action, in writing, if at any time I wish to withdraw my consent for participation in any given activity/ project.
  6. I hereby apply to become a member of Youth Action Ltd. I understand membership is free; however I do agree to pay £1.00 GBP if in the unlikely event that Youth Action closes down.

    7. I agree to the Terms and Conditions of Youth as described in the Member Declaration.
